Affordable, Insurance-Friendly Care for Complex Needs

Managing complex health conditions shouldn’t be a luxury. We believe higher-touch, condition-informed care should still be accessible.

That’s why we’ve built a model that combines insurance coverage with a low monthly membership fee, so you can get the support you need without the high price tag of concierge care. We accept insurance to help cover the cost of your visits, labs, medications, and more.

How It Works

Chronius is a membership-based primary care practice.

  • Your membership subscription gives you access to your care team and includes high-touch care coordination, patient advocacy, and longer visit lengths.

  • Your insurance is billed for medical visits, labs, imaging, and prescriptions, just like at a traditional PCP office. We bill visits as primary care, so your usual primary care coverage (deductible/copay/coinsurance) applies.

Commercial plans: Aetna, Blue Cross Blue Shield, Cigna, UHC

Medicare (traditional Medicare Part B)

Medicare Advantage plans: Aetna, BCBS, soon: UHC, Humana

Note: Not every Chronius clinician is in-network with the same plans. For more detail, see below.

Also, we cannot see patients with an HMO plan or an assigned PCP, even as out-of-network / cash pay. Such plans prevent our team from delivering the level of care coordination that our patients expect and deserve.

Not In-Network Yet? You Can Still See Us.

If we’re not in-network with your insurance yet — or if you don’t have insurance — you can still get care with us*. Here are our cash-pay rates:

  • New Patient visits: $239

  • Established Patient visits: $89–$169, depending on the length and complexity of the visit.

Even if we’re not in-network yet, you can still use your insurance for:

  • Medications filled at your preferred pharmacy

  • Labs, imaging, or other testing your Chronius provider orders

*Note: we cannot see patients with an HMO plan or an assigned PCP, even as out-of-network / cash pay. Such plans prevent our team from delivering the level of care coordination that our patients expect and deserve.

Blue Cross Blue Shield (“Florida Blue”): Commercial PPO plans. Also, BCBS Medicare Advantage PPO plans. We are not in-network with any HMO plans or any Dual Special Needs (D-SNP) plans and cannot see patients with these plans even as out-of-network / cash pay. If you have a hybrid plan type like an EPO, reach out to hello@chroniushealth.com to ask if we are in-network. Visits with Dr. Leah Probst specifically are not yet covered by BCBS.

UHC: Commercial PPO plans. Also, BCBS Medicare Advantage PPO plans (the only exception is that we are not in-network with any Student Health PPO plans). We are not in-network with any HMO plans or any Dual Special Needs (D-SNP) plans and cannot see patients with these plans even as out-of-network / cash pay. Cigna: Open Access Plus, Choice Fund PPO, PPO, ChoiceFund OA Plus, OAP with ChoiceFund, OA Plus, Carelink Planks: Open Access Plus, CIGNA Healthcare Seamless Network - Florida. We are not in-network with any HMO plans or any Dual Special Needs (D-SNP) plans and cannot see patients with these plans even as out-of-network / cash pay. We are not yet in-network with any Cigna Medicare Advantage plans. Visits with Dr. Leah Probst specifically are not yet covered by Cigna.
